Customer Identification Requirements

When you apply for a product you must complete a customer identification form and provide the required identification documents, unless you have previously done so. Full instructions are provided in the customer ID forms below.

This is to comply with our obligations under the Anti-Money Laundering and Counter-Terrorism Financing Act (2006) that requires financial services providers and financial advisers to collect and verify the identity of customers.

Advisers submitting applications on behalf of their clients must either complete the ‘record of verification procedure’ section in the ID forms or send in certified copies of customer ID documents.

Customer ID requirements for individuals/sole traders have been incorporated into both our printed and web-based product disclosure statements. You can download and print customer ID forms for other entities below.

We may request that you provide updated customer identification information at a future time and will contact you if this is required.

In the section ‘How to certify your documents’, the forms refer to the Statutory Declarations Regulations 1993. These regulations have been replaced by the new Statutory Declarations 2018 which includes a number of additional occupations/persons authorised to witness Commonwealth statutory declarations.
